If your injury is due to a Work or Auto accident, please print and complete one of the following sets of forms in addition to the New patient Forms
Worker's Compensation Forms
Most health insurance plans offer chiropractic coverage
both in and out of network. We will check your insurance
benefits for you and file electronically on your behalf.
We currently partcipate in-network with the following health
insurances:
Aetna, Blue Cross Blue Shield, Independence Blue Cross, Keystone, Amerihealth, United Healthcare, Cigna, and Medicare (Part B).
No Insurance...No problem. We have very affordable self-pay treatment plans.
Injuries resulting from auto accidents and work injuries are typically covered by your auto insurance and workers compensation policies respectively.
